Malcanthet is a demon lord, called Queen of succubi, described as an incredibly beautiful human-like female with bat-wings, horns, and a long prehensile tail tipped with a spike-like stinger.

Malcanthet’s Relationships

She is the current holder of the title of succubus Queen; other demon lords aspiring to the position include Lady Lynkhab, the currently imprisoned Shami-Amourae, and the exiled Xinivrae. Other rivals exist elsewhere in the Abyss, but they ultimately pose little or no threat to Malcanthet.

She was among the first of the succubi to come into being in the Abyss. She obtained her current title after years of waging wars and committing savage betrayals upon her fellow “sister” succubi and former paramours. As a result of her ruthless Climb to the top, she has cultivated a long list of enemies, which includes Graz’zt, Yeenoghu, and Baphomet. Her few allies include the Prince of Demons Demogorgon, with whom she birthed the monstrous Arendagrost; Pazuzu, from whom she mothered the succubus Red Shroud; her current consort Mastiphal the Hunting Sovereign; and Socothbenoth, a demon lord concerned with sexual taboos and perversions.

Malcanthet’s Realm

The succubus Queen’s realm is the 570th layer of the Abyss, and is known as Shendilavri. Once a disgusting layer of heaving and “shuddering flesh”, Malcanthet has transformed it into a verdant paradise, but ultimately, it is a paradise for only Malcanthet herself.

Cult of Malcanthet

The cult of the succubus Queen is a widespread but disorganized. Each cult sees another not as an ally but as competition, and most are little more than simple brothels. Most of Malcanthet’s followers are female; those few male cultists are viewed either as lowly servants or convenient muscle power. The Thralls of Malcanthet are exclusively female, and as they rise in power, they gain more and more powers similar to those of succubi until they too, can (temporarily) assume succubi forms.

Malcanthet’s Servants

The Radiant Sisters – Thirteen lilitu bards who serve under their mistress. Their names are actually the colors they radiate, and each one has a soul-draining locket and a shining halo.

Intoxicating Presence. If Malcanthet has not dealt damage to a creature in the past hour, that creature suffers disadvantage on any attacks against her.

Legendary Resistance (3/day). If Malcanthet fails a saving throw, she can choose to succeed instead.

Magic Resistance. Malcanthet has advantage on saving throws against spells and other magical effects.

Magic Weapons. Malcanthet’s weapon attacks are magical.

Telepathic Bond. She ignores the range restriction on her telepathy when communicating with a creature she has charmed. The two don’t even need to be on the same plane of existence.

Shapechanger. Malcanthet can use a bonus action to polymorph into any Small, Medium or Large creature, or back into her true form. Without wings, she loses her flying speed. Other than her size and speed, her statistics are the same in each form . Any equipment she is wearing or carrying isn’t transformed. She reverts to her true form if she dies.

Innate Spellcasting. Malcanthet’s spellcasting ability is Charisma (spell save DC 24). She can innately cast the following spells as 9th level spells, requiring no material components:
At will: detect magic, detect thoughts, dispel magic, hold monster, mirage arcanetelekinesis, teleport, unhallow

ACTIONS

Multiattack Malcanthet makes three attacks, two with Lover’s Lash and one with her claw. If she is disarmed, she can make two claw attacks.

Claw. Melee Weapon Attack: +14 to hit, reach 5 ft., one target. Hit: 17 (3d6 + 7) slashing damage.

Lover’s Lash. Melee Weapon Attack: +17 to hit, reach 15 ft., one target. Hit: 14 (1d8 + 10) slashing damage and 40 (9d8) electricity damage. The target must make a DC 24 Constitution saving throw or its hit point maximum is reduced by an amount equal to the damage taken. This reduction lasts until the target both finishes a long rest and receives a greater restoration or more powerful healing magic. The target is placed in permanent suspended animation if this effect reduces its hit point maximum to 0.

Tail. Melee Weapon Attack: +14 to hit, reach 5 ft., one target. Hit: 17 (3d6 + 7) bludgeoning damage.

Charm. Up to three creatures Malcanthet can see within 120 feet of her must succeed on a DC 22 Wisdom saving throw or be magically charmed. The charmed target obeys Malcanthet’s verbal or telepathic commands. If the target suffers any harm or receives a suicidal command, it can repeat the saving throw, ending the effect on a success. If the target successfully saves against the effect, or if the effect on it ends, the target is immune to Malcanthet’s Charm for the next 24 hours. Malcanthet can have any number of targets charmed at a time.

Draining Kiss. Malcanthet kisses a creature. The target must make a DC 22 Constitution saving throw against this magic, taking 117 (18d10 + 18) psychic damage on a failed save, or half as much damage on a successful one. A creature that is charmed or willing suffers disadvantage on the save. The target’s hit point maximum is reduced by an amount equal to the damage taken. This reduction lasts until the target finishes a long rest and receives a greater restoration or more powerful healing magic. The target is placed in permanent suspended animation if this effect reduces its hit point maximum to 0.

Malcanthet may blow the kiss to a creature within 60 ft. of her, the attack deals half normal damage if she does so.

Etherealness. As a bonus action, Malcanthet magically enters the Ethereal Plane from the Material Plane, or vice versa.

LEGENDARY ACTIONS

Malcanthet can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature’s turn. Malcanthet regains spent legendary actions at the start of her turn.

Call Demons. Malcanthet summons a lilitu and six succubi with maximum hit points.

Constrict. Malcanthet makes a tail attack against one foe in reach. If successful, the creature must succeed on a Strength saving throw DC 18 or become restrained. Malcanthet’s tail attack can only target the restrained creature, but she can end the effect at any time.

Stunning Beauty (Costs 3 Actions). Any creature that has damaged Malcanthet in the past round must succeed on a Wisdom saving throw DC 24 or be stunned for 1 round.

Call Demons (Sp): 3/day,Malcanthet can call 3d6 lilitu, 6d6 succubi or 3d10 incubi. These demons called by Malcanthet have double hit dice and maximum hit points.

Draining Kiss (Su): Malcanthet can bestow 2d4 negative levels with her kiss. She must succeed on a grapple check to kiss an unwilling victim. She may also blow a kiss at an enemy, bestowing 1d4 negative levels and requiring a ranged touch attack.

Poison (Ex): Fortitude DC 44; 4d6 Wisdom initial, domination secondary.

Queen’s Pleasure (Su): Malcanthet can couple with a willing or helpless living creature for an hour and grant it a +10 profane bonus to an ability score of its choice and a +10 profane bonus on saving throws. (Alternately, as a standard action requiring only a simple embrace or kiss, the bonuses are +4) The affected creature radiates chaos and evil as if its alignment were chaotic evil. Moreover, its natural weapons, as well as any weapons it wields, are treated as chaotic and evil aligned for the purpose of bypassing damage reduction.

As long as the character possesses the gift, Malcanthet’s symbol appears as a tattoo somewhere on the character’s body. The recipient cannot see its own tattoo. Malcanthet can monitor the character’s condition and location as if she had placed a status spell on the character. She can communicate telepathically with the character at all times, despite any intervening distance (even across planes), and by concentrating, can observe the world around the character as if she were there in its place. While concentrating in this manner, Malcanthet can use any of her spell-like abilities through the recipient so that they manifest as if the recipient had used the abilities itself.

As a free action, Malcanthet can cause the mark to tear bloodily away from the recipient and fade, leaving an ugly red scar. This deals 3d6 points of Charisma drain to the creature and removes the bonuses granted. However, it does not disrupt Malcanthet’s connection to the creature. Removing the mark or the scar is only possible with a dispel chaos or dispel evil spell.

Accepting a Malcanthet’s Pleasure is a chaotic and evil act. A creature can resist gaining the mark by making a Will save (DC 51 negates).

Seductive Gaze (Su): 60 ft., Will DC 51. Those who meet Malcanthet’s gaze suffer a -10 penalty to attack rolls, weapon damage, saving throws, skill checks, ability checks and level checks for 1 minute. Once per round as a free action, Malcanthet can attempt to dominate a creature that is currently suffering from her seductive gaze (Will DC 51 negates). Malcanthet can dominate up to six creatures at a time with this ability; if she attempts to dominate a seventh, the first creature she dominated is immediately freed from the effect. The domination otherwise persists until removed with break enchantment, wish or miracle, it cannot be dispelled.

Sneak Attack: Malcanthet deals +26d6 points of damage and 2 points of Strength damage on a successful sneak attack. The damage is repeated the following round. Malcanthet can forgo 2d6 points of sneak attack damage and cause a bleeding wound that loses 1 hit point per round until healed.

Cosmic Entity: Malcanthet has a +3 bonus on rank checks. She has a +15 bonus on rank checks to overcome immunity to mind-affecting and energy drain.

Profane Beauty (Su): Good aligned creatures that touch or make melee attacks against Malcanthet must make a Fortitude save (DC 51) each time or gain 2 negative levels. Malcanthet heals 20 hit points each time her profane beauty bestows negative levels.

Spell Reflection (Su): Whenever a spell fails to penetrate Malcanthet’s Spell Resistance, it is reflected back on the caster as if Malcanthet had cast it. The spell uses its original caster level and DC.

Possessions: Lover’s Lash is a +6 adamantine scourge of shocking blast. All damage dealt by this weapon is vile (including sneak attack damage, if used to make one). On a critical hit, the damage is permanent and deals a horrible wound that must be removed with wish or miracle before the lost hit points can be restored. A creature slain by the lash is placed into suspended animation just prior to death, similar to temporal stasis. However, the creature languishes in the eternal agony of the deathblow, completely aware of its surroundings and the passage of time. A freedom spell is required to release the victim, who immediately dies and can be restored to life through true resurrection.

Malcanthet also wears various enchanted jewelry and gowns.
