Salus is the Roman goddess of public welfare, health and safety. Her concern is not merely curing one sick person: it is whether the community remains capable of protecting life, maintaining clean water, preventing avoidable disaster and recovering when systems fail.
Divine Mechanics
Salus is presented as a true deity. The 5.5e and Pathfinder statistics are deliberate epic campaign extensions, so CR and ability scores are not capped at ordinary published monster limits.

Actions
Multiattack. Salus makes three Serpent Staff attacks.
Serpent Staff. Melee or Ranged Spell Attack: +21 to hit, reach 10 ft. or range 120 ft., one target. Hit: 28 (4d8 + 10) force plus 22 (4d10) radiant damage.
Restore the Community (Recharge 5–6). Up to six creatures Salus chooses within 90 feet regain 45 Hit Points and may end one Charmed, Frightened, Poisoned or Diseased condition affecting them.
Reaction
Safeguard. When a creature within 60 feet takes damage, Salus grants it Resistance to the triggering damage and 20 Temporary Hit Points.
Legendary Actions
Salus can take 3 Legendary Actions per round.
- Move. Move up to half Speed without provoking Opportunity Attacks.
- Serpent Staff. Make one Serpent Staff attack.
- Portfolio Intervention (Costs 2). Use one non-damaging portfolio ability, domain spell of 5th level or lower, or protective effect without expending a normal action.

Salus in Roman Religion
Salus concerns the safety and welfare of the Roman people and state. Her temple stood on the Quirinal and her festival was observed on 5 August. Later imagery and religious development make health increasingly visible, but public well-being remains the stronger starting point for understanding her.
What Is Salus Doing Right Now?
Salus watches cities threatened not only by disease but by famine, unsafe water, collapsing defences, reckless rulers and systems that sacrifice the many for the comfort of a few. Her concern is whether a community can continue to live.
Her signs appear as clean water where contamination was expected, a serpent resting peacefully beside a public fountain, or an official suddenly understanding the human cost hidden inside a ledger.
Worship in the Campaign Present
Salus belongs wherever public safety becomes a sacred responsibility. Healers, water wardens, granary officials, builders, city councils and people responsible for evacuation or quarantine all invoke her when private convenience threatens collective survival.
Her rites favour clean water, shared cups, serpent imagery and public accounting of hazards. A priest of Salus may spend more time inspecting a bridge, cistern or grain store than treating an individual wound.

Attunement: Required by a creature formally responsible for the welfare, sanitation or safety of a settlement.

The Serpent Wakes. The carved serpent moves and raises its head when an avoidable threat capable of harming at least 100 residents is present within 1 mile. It indicates the general direction but not the exact nature of the danger.
Public Remedy (3/Day). One creature you touch regains 4d8 + 8 Hit Points or repeats one saving throw against Poisoned or disease with Advantage.
Ward the Community (1/Week). For 24 hours, creatures within a 300-foot-radius public shelter gain Advantage on saves against mundane disease and poison.
Pathfinder 1e
Major Divine Relic · CL 18th. Warns of avoidable mass danger within 1 mile. 3/day: cure critical wounds or grant a new save against poison/disease with +4 sacred bonus; 1/week: protect a 300-foot public shelter from mundane disease and poison for 24 hours.

Breaking the Staff
The Staff loses its magic if its attuned keeper knowingly conceals an avoidable mass danger to preserve their own office, wealth or reputation until residents are harmed.
The Cup of Public Health
Lesser Divine Relic · Wondrous Item (cup)
Water poured into the Cup visibly clouds or discolours in the presence of mundane poison, disease-causing contamination or deliberate adulteration. It identifies danger but does not itself purify the water.
Adventure Hooks
- A city is free of plague, yet Salus’s priests insist its public welfare is in mortal danger.
- A miraculous spring heals individuals while slowly poisoning the river downstream.
- A ruler suppresses warnings from Salus because evacuation would ruin an important festival.
- The Quirinal Serpent Staff wakes and points toward a perfectly ordinary government archive.

Divine Estate and Regional Effects
Where Salus’s presence is strong, a major sanctuary can project these effects across roughly six miles. A household or neighbourhood cult manifests them on a much smaller scale. Unless another divine power contests them, they fade over 1d10 days after the sanctuary is abandoned or desecrated.
Water Gives Warning
Public fountains and wells under Salus’s protection visibly cloud before contaminated water is drawn.
Panic Does Not Spread
Creatures within a consecrated refuge have Advantage on saving throws against fear caused by disaster, disease or crowd panic.
Neglect Becomes Visible
Unsafe walls, rotten timbers, spoiled provisions and other preventable civic hazards show conspicuous cracks, smells or stains before they cause a catastrophe.
