She is both revered and feared by the living, who know that they too will one day cross over into her domain. Her role as the guardian of the dead gives her immense power and influence, as she is able to grant or withhold entry to the afterlife as she sees fit.

Divine Power. Tuonetar can cast spells as if she were a 40th-level spellcaster. Her spellcasting ability is Intelligence (spell save DC 28, +20 to hit with spell attacks). She can use her Divine Power to cast the following spells at will:

  • Control Undead, Ray of Enfeeblement, Ray of Sickness, Vampiric Touch

Legendary Resistance (5/Day). If Tuonetar fails a saving throw, she can choose to succeed instead.

Magic Resistance. Tuonetar has advantage on saving throws against spells and other magical effects.

Regeneration. At the start of each of Tuonetar’s turns, she regains 50 hit points if she has at least 1 hit point remaining.

Actions

Multiattack. Tuonetar can use her Frightful Presence. She then makes three attacks: one with her staff and two with her touch.

Staff. Melee Weapon Attack: +25 to hit, reach 10 ft., one target. Hit: 40 (6d10+10) bludgeoning damage plus 40 (12d6) necrotic damage.

Touch. Melee Spell Attack: +20 to hit, reach 10 ft., one target. Hit: 45 (10d8) necrotic damage. The target’s hit point maximum is reduced by an amount equal to the damage taken. This reduction lasts until the target finishes a long rest. The target dies if this effect reduces its hit point maximum to 0.

Frightful Presence. Each creature of Tuonetar’s choice within 120 feet of her and aware of her must succeed on a DC 28 Wisdom saving throw or become frightened for 1 minute. A creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success. If a creature’s saving throw is successful or the effect ends for it, the creature is immune to Tuonetar’s Frightful Presence for the next 24 hours.

Divine Aura. Tuonetar’s presence inflicts fear on all creatures within 120 feet of her. Each creature in that area must make a DC 28 Wisdom saving throw. On a failed save, a creature is frightened for 1 minute. A creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.

Legendary Actions

Tuonetar can take 3 legendary actions per turn, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature’s turn. Tuonetar regains spent legendary actions at the start of her turn.

Cast a Spell. Tuonetar casts a spell from her list of spells at will.

Attack. Tuonetar makes one attack with her staff or touch.

Frightful Presence (Costs 2 Actions). Tuonetar uses her Frightful Presence.

Divine Presence (Costs 3 Actions). Tuonetar radiates divine power from her body. All creatures within 60 feet of her must make a DC 28 Wisdom saving throw or take 50 (10d10) necrotic damage and be frightened for 1 minute. On a successful save, a creature takes half as much damage and is not frightened.

The Key to Tuonela

Artifact (requires attunement by a creature that Tuonetar designates)

The Key to Tuonela is a powerful artifact in the form of a black, bone-like key that radiates a dark energy. It is said to be the only key that can unlock the gates of Tuonela, the Finnish underworld, where Tuonetar resides.

Properties:

  • The Key to Tuonela grants the attuned creature the ability to cast the following spells at will: Darkness, Ray of Enfeeblement, Ray of Sickness, Vampiric Touch.
  • The Key to Tuonela has 5 charges, and it regains 1d4+1 expended charges daily at dawn.
  • The attuned creature can use an action to expend 1 charge and create a magical portal to Tuonela within 30 feet of it. The portal lasts for 1 hour or until it is used.
  • The attuned creature can use an action to expend 3 charges and open a portal to Tuonela that any creature within 60 feet of the attuned creature can enter. The portal lasts for 1 hour or until it is used.
  • The attuned creature can use an action to expend 5 charges and summon Tuonetar herself to the location of the attuned creature. Tuonetar arrives within 1 minute and will stay for up to 1 hour or until she is dismissed by the attuned creature. While Tuonetar is present, she is under the attuned creature’s control, but she will not act against her own interests.
  • If the Key to Tuonela is used to open a portal to Tuonela, any creature that passes through it without the attuned creature’s permission will be subject to Tuonetar’s wrath.
  • If the Key to Tuonela is ever destroyed, Tuonetar will know immediately and will likely seek revenge on the one responsible.

Note: The Key to Tuonela is an artifact of immense power and should only be used in a campaign where the characters are of a high level and are facing similarly powerful enemies.
