Daemon, Anthraxus “Oinodaemon”
“Meet Anthraxus the Oinodaemon: the diseased overlord of the Lower Planes whose touch spreads death and whose schemes could topple gods!”
Anthraxus is a towering humanoid figure of nearly ten feet, with a skeletal, diseased ram’s head that exudes an aura of death and pestilence. His body is covered in necrotic flesh, riddled with open sores and oozing wounds, with the texture of his skin shifting between leathery rot and brittle decay. His appearance is both regal and revolting, as he dresses in a tattered grey robe, once the symbol of his rulership, now a testament to his fall from power.
Behavior
Cold, calculating, and deeply manipulative, Anthraxus is a master of deception and treachery. He rarely acts without considering the long-term consequences of his actions, preferring to orchestrate events from the shadows rather than act directly. When he does intervene, his actions are decisive and devastating. He maintains an aura of dread authority, using intimidation, fear, and his vast intellect to dominate those around him.
Habitat
Anthraxus is closely tied to Oinos, the first layer of the Gray Waste of Hades, a plane of utter despair and hopelessness. Oinos is a diseased wasteland where the very environment drains life and color from all who enter. As a former ruler of the Wasting Tower, a towering fortress in this realm, Anthraxus once commanded all yugoloths from this seat of power. Though he was eventually deposed, the plane itself remains indelibly marked by his influence.
Modus Operandi
Anthraxus wields incredible magical and physical power, often using them to inflict sickness and destruction. His most iconic weapon is the Staff of the Lower Planes, a relic capable of controlling minds, summoning fiends, and spreading devastation. His strategies are a mix of direct displays of strengthâinflicting uncurable diseases on his enemiesâand subtle manipulation, such as making contracts or deals that ultimately unravel his rivals.
He excels at exploiting weakness, offering aid or power to desperate beings, only to twist their reliance into servitude. His presence in a campaign often brings widespread suffering, as his machinations involve corrupting mortals, sowing chaos, and spreading plagues.
Motivation
Anthraxus is motivated by an insatiable hunger for power and a deep-seated desire for revenge. Having been dethroned as Oinoloth, he yearns to reclaim his former position and restore his control over the yugoloths. Beyond personal ambitions, Anthraxus revels in the destruction and despair he sows, seeing disease and decay as his ultimate tools to unravel both the mortal and divine. His endgame is a multiverse steeped in entropy, a vision of a world slowly succumbing to inevitable rot.
As a villain, Anthraxus embodies the slow, inexorable spread of corruption, making him a terrifying presence in any campaign. Whether through direct confrontation or the lingering effects of his schemes, his influence is both pervasive and devastating.

Role in Campaigns
Anthraxus is a world-ending force, summoned by dark rites or an unintended consequence of meddling with ancient magics. A perfect final boss for campaigns involving corruption, decay, or apocalyptic threats, it can serve as an unstoppable plague bearer, forcing adventurers to find creative ways to combat the embodiment of decay itself.
Plaguebearers (Summoned Minions)
Use the Oozing Horror stat block with modifications for a rotting appearance and necrotic damage resistance.
Lore
Anthraxus, the Oinodaemon, is an ancient primordial force that embodies the inevitable decay of all things. Once a celestial being, it now exists to unravel the very fabric of existence. Manifesting as a towering, skeletal figure crowned with spiraling ram’s horns, Anthraxus exudes an aura of death and decay, rotting the land it treads upon. Its glowing green eyes pierce through time itself, seeing the entropy that will eventually consume all.
