Pain Domain

Ritual ordeal representing the Pain Domain

The Pain Domain is a dark D&D 3.x sphere in which suffering is treated as power, weapon and sacrament. It originated in Book of Vile Darkness and later entered 3.5-era Forgotten Realms material, where Loviatar is among the deities associated with it.

Edition Applicability

Pain originated in a D&D 3.0 source and continued to be used in 3.5-era material. It is therefore labelled here as D&D 3.x rather than falsely presenting the original text as a 3.5 SRD domain. No Pathfinder or 5e tab is added.

Pain Domain — D&D 3.x

Granted Power

Once per day, you can convert damage you deal in one blow into healing for yourself, up to a maximum amount of healing equal to your level.

Pain Domain Spells

1st Angry Ache
2nd Sadism
3rd Wrack
4th Liquid Pain
5th Thousand Needles
6th Pox
7th Wave of Pain
8th Symbol of Pain
9th Eternity of Torture

3.5-era rules note: published errata for Book of Vile Darkness replaced the 8th-level domain spell with power word stun, with widened symbol of pain offered as an alternative. A table using the 3.5 update should apply that correction.

How the Pain Domain Plays

Pain is an offensive debuff domain. Its spells impose penalties, helplessness, ability damage and other severe conditions, while the granted power lets a cleric turn one damaging strike into limited self-healing.

Pain and Suffering

The Pain Domain is about actively inflicting or mastering suffering. That is different from a protective or healing faith whose priests simply endure hardship. Deity, alignment and campaign context determine whether the domain appears as a villainous cult, a harsh trial tradition or something else.

Priests, Cults and Sacred Duties

Priests of Pain tend to interpret suffering as revelation, discipline, punishment or domination. In darker traditions they deliberately inflict it; other traditions may focus on endurance and ritual ordeal. The rules themselves are overtly severe and should be used in a campaign that wants that tone.
