Royal Naga — Five-Headed Serpent Guardian

A five-headed naga sovereign whose layered gazes, sorcery and carefully chosen disguises make it a terrifying guardian of forgotten kingdoms.

The Royal Naga is among the most imposing members of the naga family: one enormous serpent body supporting five hooded necks, each ending in a sharp-featured humanoid face. The five heads are not decorative. Together they watch multiple directions, maintain several conversations at once and focus supernatural gazes that can rob an intruder of sight or hearing.

Run It Tonight

A royal naga should be encountered as a ruler before it is encountered as a monster. One head questions the party, another watches the rear ranks, a third studies magic, and the remaining two keep track of exits and hidden creatures. If combat starts, its gaze should immediately change how the party positions itself.

Traits

Fivefold Awareness. The royal naga cannot be Surprised while conscious and has Advantage on Wisdom (Perception) checks.

Five Faces. The naga can magically assume one of five specific Medium Humanoid forms it knows. Each form always has the same appearance. The naga’s statistics do not otherwise change, and it can still use its Gaze abilities.

Spellcasting. The royal naga uses Charisma for spellcasting (spell save DC 16, +8 to hit). It can cast: at will—detect magic, message, minor illusion; 3/day each—blink, dispel magic, suggestion; 2/day each—arcane eye, greater invisibility; 1/day—charm monster.

Actions

Multiattack. The royal naga makes three Bite attacks.

Bite. Melee Attack Roll: +10, reach 15 ft. Hit: 15 (2d8 + 6) Piercing damage plus 3 (1d6) ongoing Bleed damage at the start of the target’s turns until it or another creature succeeds on a DC 15 Wisdom (Medicine) check or magical healing is received.

Rend. If three Bite attacks hit the same creature on one turn, the target takes an extra 16 (3d6 + 6) Slashing damage.

Dual Gaze. The naga chooses one gaze and one creature it can see within 60 feet. The target makes a DC 16 Constitution saving throw. Blinding Gaze: on a failure, the target is Blinded for 1 minute. Deafening Gaze: on a failure, the target is Deafened for 1 minute. The target repeats the save at the end of each turn, ending the effect on a success.

Dual Gaze. One creature within 60 feet that can see the naga must make a DC 16 Constitution saving throw or become blinded or deafened (naga’s choice) for 1 minute. The creature repeats the save at the end of each turn.

The royal naga can assume one of five fixed humanoid identities. This is a compatibility conversion of the Pathfinder ability.

Dual Gaze (Su)

The naga can use a gaze that causes blindness or deafness; Fortitude DC 21 negates. Treat the effect as permanent until cured, matching the legacy PF presentation.

The royal naga casts spells as a 9th-level sorcerer.

Behaviour

Royal nagas are proud and formal. They enjoy conversation but tend to speak as rulers, judges or hosts rather than equals. Their five heads may disagree over emphasis or strategy, but they remain one creature with one purpose rather than five independent personalities.

Signs Before Initiative

  • Five different humanoid portraits appear repeatedly in a ruined palace despite being painted centuries apart.
  • Treasure is arranged ceremonially rather than piled randomly.
  • Intruders who escaped the ruin describe losing hearing before anyone was attacked.
  • Invisible scouts are discovered with unnerving reliability.
  • Five voices answer from different parts of a dark chamber, but only one body is present.
  • Jewelled crowns and facial ornaments fit no ordinary humanoid anatomy.

If Violence Starts

The naga uses its gaze before committing all five heads to melee. Deafening enemies first disrupts communication and spellcasting cues; blinding those who continue to advance then makes the bite barrage far more dangerous. It uses magic to isolate or delay anyone who remains effective at range.

Counterplay

Avoiding the gaze or breaking line of sight is the first priority. Blind characters are no longer useful targets for sight-based gaze effects, but they have their own obvious disadvantages, so protective magic and restorative resources matter greatly.

Do not assume one humanoid disguise means one identity. A royal naga has five recurring forms and can use them over decades to maintain several political relationships at once.

Using a Royal Naga in an Encounter

The five ambassadors. Five respected advisers in different courts are discovered to be the same royal naga using separate fixed identities.

The silent palace. Every survivor of a treasure expedition has permanently lost their hearing but refuses to describe what they saw.

The inherited crown. A royal naga claims that a relic in the party’s possession belongs to the kingdom it has guarded for centuries.

The fifth face. Four of the naga’s humanoid identities are known. Nobody has yet worked out who the fifth one is.
