Appearance: The Belier Devil, also known as Bdellavritra, is a large creature that resembles a grotesque fusion of various beings. This body is topped with a leech-like mouth, a horrifying sight that is used to drain the life force of its victims. At the opposite end, the creature features a knot of three human heads, each capable of independent thought and speech. Despite its large size, the Belier Devil weighs around 3,000 pounds, allowing it to move with surprising agility.

Behavior: The Belier Devil is a master manipulator, preferring to control from the shadows rather than engage in direct combat. It is known to possess influential individuals, using them as puppets to orchestrate events according to its nefarious plans. If its schemes are discovered, the Belier Devil does not hesitate to use the full power of its host body to retaliate, demonstrating its aggressive nature when threatened.

Habitat: The Belier Devil is an extraplanar entity, making its home in the infernal planes of Hell. It thrives in any environment within this realm, from the fiery pits to the icy wastes. Solitary by nature, it is often found alone, lurking in the shadows and plotting its next move.

Modus Operandi: The Belier Devil employs a range of spell-like abilities to achieve its goals. It can cast spells such as acid arrow, greater dispel magic, greater teleport, and persistent image at will. It also has the ability to use Magic Jar to possess others, a process that does not require a receptacle and renders its own body ethereal. This possession ability is particularly feared, as it allows the Belier Devil to control its victims with terrifying precision.

Motivation: The primary motivation of the Belier Devil is control. It seeks to manipulate events and individuals to serve its own ends. Despite its fearsome reputation, it does not seek conflict for conflict’s sake. Actions

Multiattack. The Belier Devil makes four attacks: one with its bite and three with its tongues.

Bite. Melee Weapon Attack: +23 to hit, reach 10 ft., one target. Hit: 25 (4d8 + 7) piercing damage.

Tongue. Melee Weapon Attack: +23 to hit, reach 20 ft., one target. Hit: 14 (2d6 + 7) slashing damage, and the target is grappled (escape DC 25). Until this grapple ends, the target is restrained, and the Belier Devil can’t use that tongue on another target. If the target is a creature, it must succeed on a DC 22 Constitution saving throw or be paralyzed for 1 minute. The target can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.

Blood Drain. Melee Weapon Attack: Automatically hits a grappled target. Hit: The target takes 1d4 Constitution damage as the Belier Devil drains its blood.

Constrict. When the Belier Devil successfully grapples a creature, it can deal 14 (1d6 + 7) bludgeoning damage to the grappled creature.

Infernal Charge (Recharge 5-6). The Belier Devil moves up to 30 feet in a straight line and can move through other creatures and objects as if they were difficult terrain. The first time it enters a creature’s space, the creature must make a DC 18 Strength saving throw. On a failed save, the creature takes 21 (6d6) bludgeoning damage and is knocked prone. On a successful save, the creature takes half as much damage and isn’t knocked prone.

If it uses Magic Jar on a host while on the Material Plane, its own body becomes ethereal for the duration of the possession, and the Magic Jar effect lasts until dispelled, the devil ends the effect, it is forced out, or it or its host body is slain.
  • Masterful Manipulators: These devils avoid physical confrontation, preferring to manipulate events from behind the scenes. If their schemes are uncovered, they retaliate with all the power at their host bodies’ disposal.
  • Spellcasting: In combat, the Belier Devil uses its spell-like abilities to control and weaken its enemies. It employs dominate person and mass suggestion to turn foes against each other and greater dispel magic to counter magical threats.
  • Physical Confrontation: If forced into physical combat, the Belier Devil uses its Infernal Charge to break enemy lines, then grapples and drains the blood of weakened foes with its tongues.

Masterful possessors and foul manipulators, belier devils are among the most feared masterminds of Hell. Known as bdellavritras to devilkind, these worm-like fiends avoid physical confrontation. Instead, they target influential individuals for possession, using them to manipulate events by proxy. If their schemes are uncovered, the devils retaliate with all the power at their host bodies’ disposal.

A bdellavritra’s voice can emanate from its worm-like maw, from any of its human-like mouths, or from all four at once, as it desires. Each mouth has its own distinct voice, often pleasing to the human ear. Bdellavritras typically reach lengths of 16 feet from worm-mouth to human heads, and weigh 3,000 pounds.
