5.5e / 2024, 5e / 2014 and 3.5e

No mechanics are presented here as official rules for these editions. A conversion should preserve the unusual bargain: increased vulnerability to sleep and immediate fatigue in exchange for vivid dreams and unusually efficient restorative rest.

What Happens in Play?

Midnight milk is useful because sleep itself becomes part of the encounter. A character who remains awake suffers the drawbacks without receiving the restorative benefit; one who sleeps becomes vulnerable while unconscious but may awaken dramatically refreshed.

Addiction & After-effects

The PF1e version carries moderate addiction and Wisdom damage. Repeated doses also lengthen the user’s vivid dream periods, making accumulated use visible in play rather than merely existing on a character sheet.

Encounters & Adventure Use

  • A guide recommends midnight milk before a journey that allows almost no time for rest.
  • A sleeper cannot easily be awakened while danger approaches.
  • A cult uses the draught to induce shared nights of vivid dreaming.
  • An exhausted adventurer must choose between remaining alert and taking the only chance for restorative sleep.
  • A recurring dream produced by repeated doses contains a clue that changes each night.
