Restored form cancels any polymorph or wild shape effects, as well as similar physical changes (such as those caused by alterself). This spell even restores petrified creatures back to living flesh, and undoes the effects of a withering curse. It is not necessary for you to know the subject’s name or native form for this spell to work. Note that this spell works on both voluntary and involuntary transformations. This spell does not prevent further transformations – for instance, a lycanthrope forced into its true form may shape shift into another form on its next action.
Material Component: A sliver of broken mirror.
