“Nyogoth: The Abyssal Horror That Devours Everything in Its Path!”
Appearance: The Nyogoth is a grotesque, floating mass of tangled intestines encircling a central, gaping maw lined with razor-sharp teeth. Each of its coiling tendrils terminates in smaller, toothy mouths, and its body is filled with highly corrosive digestive fluids. It propels itself through the air with convulsive, squirming movements, resembling a nightmarish amalgamation of viscera and predatory hunger.
Behavior: Despite its horrifying appearance, the Nyogoth exhibits a rudimentary intelligence, allowing it to set cunning ambushes for unsuspecting prey. It is a relentless scavenger, feeding on the filth and remnants left behind by other denizens of the Abyss. However, it prefers the taste of living flesh and will aggressively pursue live prey when the opportunity arises.
Habitat: Nyogoths inhabit the deepest reaches of the Abyss, particularly the Outer Rifts, where they serve as scavengers in the Abyssal ecosystem. They are often found lurking in areas teeming with decay and refuse, thriving in the filth and detritus that accumulate in these desolate regions.
Modus Operandi: Nyogoths employ stealth and their innate abilities to incapacitate and consume their prey. They can unleash a nauseating display by untangling their tentacles and exposing their many mouths, causing confusion and sickness in nearby creatures. When injured, their bodies spray acidic blood, harming those in close proximity. They also possess spell-like abilities, such as acid arrow, fear, and dimension door, to weaken or escape from adversaries.
Motivation: Driven by an insatiable hunger, Nyogoths are compelled to feed continuously. While they can subsist on waste and refuse, they crave the flesh of living beings. Their existence is a constant cycle of feeding and seeking new sources of sustenance, making them a perpetual threat to any creature that crosses their path.
The Nyogoth embodies the chaotic and malevolent nature of the Abyss, serving as a reminder of the horrors that lurk in its darkest depths.

Amorphous Form. The nyogoth can move through a space as narrow as 1 inch wide without squeezing.
Acidic Blood. When the nyogoth takes piercing or slashing damage, each creature within 5 feet of it must succeed on a DC 15 Dexterity saving throw or take 7 (2d6) acid damage.
Horrific Appearance. Any creature that starts its turn within 30 feet of the nyogoth and can see it must succeed on a DC 15 Wisdom saving throw or be frightened for 1 minute. A frightened target can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success. A creature that succeeds on the saving throw is immune to this nyogoth’s Horrific Appearance for the next 24 hours.
Actions
Multiattack. The nyogoth makes two Tentacle Bite attacks and one Maw Bite attack.
Tentacle Bite. Melee Weapon Attack: +7 to hit, reach 10 ft., one target. Hit: 8 (1d10 + 3) piercing damage plus 4 (1d8) acid damage.
Maw Bite. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 13 (2d8 + 3) piercing damage plus 4 (1d8) acid damage.
Nauseating Display (Recharge 5–6). The nyogoth unfurls its tentacles and reveals its many maws in a grotesque display. Each creature within 30 feet that can see the nyogoth must succeed on a DC 15 Constitution saving throw or be poisoned for 1 minute. While poisoned in this way, the creature is incapacitated. A poisoned creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.
Innate Spellcasting. The nyogoth’s innate spellcasting ability is Charisma (spell save DC 15). It can innately cast the following spells, requiring no material components:
- At will: acid arrow
- 3/day each: fear, protection from good and evil
- 1/day: acid fog, dimension door
Tactics
The nyogoth prefers to ambush prey, using its Nauseating Display to incapacitate groups before closing in with its Maw Bite. It uses acid arrow to weaken foes at range and dimension door to reposition or escape if overwhelmed. Its Acidic Blood deters melee attackers, making it a dangerous foe in close quarters.

- Horrific Appearance (Su): Creatures that succumb to a nyogoth’s horrific appearance become nauseated for 1d8 rounds.

DESCRIPTION
The nyogoth’s role in the Abyss is that of a scavenger. Essentially mobile clumps of buoyant intestines, these writhing creatures squirt through the air in convulsive movements like an octopus gliding through water. They are constantly on the search for anything smaller than one of their many mouths. They can subsist on the waste and filth left behind by other denizens of the Abyss but particularly enjoy feeding on still-living creatures.
Despite their seemingly lowly role in Abyssal ecosystems, nyogoths are far from stupid beasts. Most are nearly as intelligent as the average human and are capable of solving relatively complex problems when it comes to securing their next meal.
