Swarm of Rats, The Crawling Plague
“An unstoppable tide of gnashing teeth and filthy fur—face the nightmare that turns alleys into deathtraps: the Swarm of Rats!”

The Swarm of Rats appears as a living, writhing mass of hundreds of tiny, scrabbling rodents. Individually, the rats are unremarkable—grey, mottled fur, sharp teeth, and beady eyes—but together they form a shifting, almost fluid shape that moves with unnerving speed and purpose. The swarm’s edges are constantly in motion, flickering with twitching whiskers and snapping jaws, creating a chittering, unsettling noise that echoes from every direction.

Behaviour:
This creature operates as a single entity, coordinated not by intellect but by instinct and survival. The swarm relentlessly pursues any source of food or threat, overwhelming foes by sheer numbers and ferocity. While individual rats may scatter if separated, the swarm remains cohesive, responding to threats with frantic, biting attacks and retreating only when severely injured or starved. It avoids bright light and open spaces, preferring to strike from shadowed corners and confined passages.
Habitat:
Swarm of rats thrive in dark, damp environments rich in refuse and detritus. Commonly found in sewers, abandoned buildings, ancient ruins, and crowded urban alleys, they favor places where food waste is plentiful and predators few. Their nests are tangled masses of bones, scraps, and stolen items hidden in crevices or under debris.
Modus Operandi:
The swarm of rats attacks by rushing and enveloping its prey, using its many tiny teeth to inflict painful bites simultaneously. Its fluid movement allows it to slip through cracks and gaps to ambush victims in tight quarters. The swarm’s resilience lies in its numbers: while individual rats are fragile, the collective is difficult to damage. It often waits in the shadows, attacking suddenly and retreating to the safety of its lair when injured.
Motivation:
Driven by hunger and instinct, the swarm of rats seeks only to feed and survive. It has no true intelligence or malice but acts as a force of nature, consuming anything edible and defending its territory fiercely. In larger numbers or under unnatural influence, it can become a terrifying force, overwhelming entire groups or spreading plagues through its bite.

Traits
Swarm Form.
The Swarm of Rats can occupy another creature’s space and vice versa, and the swarm can move through any opening large enough for a Tiny rat. The swarm cannot regain hit points or gain temporary hit points.
Keen Smell.
The swarm has advantage on Wisdom (Perception) checks that rely on smell.
Swarm Resilience.
When the swarm is reduced to half its hit points or fewer, it has disadvantage on attack rolls and ability checks until healed above half its hit points.
Actions
Bites. Melee Weapon Attack: +4 to hit, reach 0 ft., one target in the swarm’s space.
Hit: 14 (4d6) piercing damage, or half damage if the swarm has half its hit points or fewer.
Infectious Frenzy (Recharge 5–6).
The swarm erupts in a frenzy of biting and scratching. Each creature in the swarm’s space must succeed on a DC 12 Constitution saving throw or be poisoned for 1 minute. A poisoned creature can repeat the saving throw at the end of each of its turns, ending the effect on a success.
Tactics
- The Swarm of Rats seeks to engulf foes in tight spaces such as tunnels, sewers, or ruined buildings, making escape difficult.
- It uses its Infectious Frenzy to weaken tougher foes early in combat.
- Prefers to remain out of reach of melee fighters, slipping through cracks and holes to bypass frontline defenders.
- If heavily damaged, it retreats to regroup or hides in shadows until it can attack again.
Environment:
Urban sewers, ruins, caves, dark alleys.

DESCRIPTION
A swarm of rats is a writhing tide of fur and teeth, driven by hunger and instinct. Though each rat is individually harmless, together they form a dangerous, nearly unstoppable mass capable of stripping flesh from bone. These swarms thrive in filth and decay, commonly found in cities, dungeons, and plague-ridden ruins.
Swarm infestations often herald deeper corruption—necromantic influence, foul cult activity, or the early signs of plague. In some dark places, these creatures are revered by vermin cultists or used as weapons by pestilence-worshiping druids and necromancers.
