Buer is a spirit that appears in the 16th century grimoire Pseudomonarchia Daemonum and its derivatives, where he is described as a Great President of Hell, having fifty legions of demons under his command. He appears when the Sun is in Sagittarius. Buer teaches Natural and Moral Philosophy, Logic, and the virtues of all herbs and plants. He also heals all infirmities, especially of men, and gives good familiars.
He is depicted in the shape of Sagittarius, which is as a centaur with a bow and arrows.
According to other authors he teaches Medicine, and has the head of a lion and five goat legs surrounding his body to walk in every direction.
Although the etymology of his name is uncertain, curiously there was an ancient city named “Buer” (now Gelsenkirchen) in Westphalia, Germany.
Buer is a Great President of Hell who manifests as a grotesquely symmetrical wheel of five lion-faced goat legs, rotating in a macabre spiral that defies natural movement. His central form is often described as a glowing core of occult symbols and smoldering fire, with radiant infernal eyes gazing out from the space where a torso should be. His voice resounds with scholarly calm, speaking in riddles laced with philosophical truths and medical doctrine.
Behavior: Buer is eerily calm, erudite, and patronizing, often assuming a mentor-like role. He speaks with patience, as if addressing dim-witted students, yet never hides his disdain for weakness or ignorance. He rarely shows overt malice, instead preferring manipulation through enlightenment, using healing and wisdom to instill loyalty or spiritual dependence.
Habitat. Buer dwells in Dis, the Iron City of Hell’s second layer, within a spiraling infirmary-fortress of obsidian and ivory known as the Helixum. Here, demonic surgeons perform infernal research into medicine, soulcraft, and pain. In the mortal world, he manifests at shadowy crossroads, desecrated hospitals, and cursed libraries, particularly during times of plague, madness, or ideological upheaval.
Modus Operandi: Buer offers forbidden knowledge, particularly in medicine, herbalism, and philosophy, in exchange for obedience or souls. He tempts mortals with cures for incurable ailments or truths that unravel sanity. When summoned, he performs miraculous healing—but always implants subtle compulsions or arcane diseases with long-term consequences. He also commands legions of demonic spirits skilled in alchemy and healing arts.
Motivation: Buer seeks to subvert divine order by proving that infernal wisdom and healing surpass celestial mercy. His ultimate goal is to reshape mortal understanding of health, sin, and virtue, turning salvation into a contract and medicine into sorcery. Every life he saves is a soul inching closer to damnation by gratitude.

Buer calls upon infernal logic to dispel madness. Each enemy within 60 feet must make a DC 26 Wisdom saving throw or be stunned for 1 minute. Affected targets can repeat the saving throw at the end of each of their turns.
Legendary Actions
Buer can take 3 legendary actions, choosing from the options below. Only one legendary action can be used at a time and only at the end of another creature’s turn. Buer regains spent legendary actions at the start of his turn.
Detect. Buer makes a Wisdom (Perception) check.
Hoof Attack. Buer makes one Flaming Hooves attack.
Suppress Malice (Costs 2 Actions). All hostile creatures within 30 feet must succeed on a DC 26 Charisma saving throw or have disadvantage on attack rolls until the end of their next turn.
Lair Actions (In Hell, Initiative Count 20, Losing Initiative Ties)
Buer can invoke the following lair actions while in his infernal sanctum:
- Flames of Purification. Friendly creatures regain 30 hp and gain resistance to fire damage for 1 minute.
- Clarity of Purpose. Enemy creatures within 60 ft. must succeed on a DC 26 Intelligence saving throw or become confused for 1 minute.
- Suppress the Unjust. All healing spells cast by non-evil aligned casters in the lair are halved until the next round.
Regional Effects
The region around Buer’s lair is warped by his presence, creating the following effects:
- Springs and fountains in the area gain minor healing properties (1 hp per minute when drinking from a natural source).
- No disease or poison can naturally exist within 6 miles of his lair.
- Fiends and celestials within the region feel a compulsion to parley before engaging in violence (DC 20 Charisma save to ignore).
If Buer is destroyed, these effects fade over 1d10 days.

SPECIAL ABILITIES
Divine Healing (Su) Buer can channel infernal energy to heal 10d8+30 damage to up to 6 targets within 120 ft. as a swift action. This also removes one condition (sickened, poisoned, exhausted, fatigued, etc.). Usable 3/day.
Infernal Enlightenment (Su) Once per day, Buer may force all creatures within 60 ft. to make a Will save (DC 42) or be affected as by dominate person for 24 hours, convinced they are following divine wisdom. This is a mind-affecting compulsion.
Ring of Reason (Su) Every 1d4 rounds, Buer may emit an aura of irresistible logic. Creatures within 60 ft. must succeed a DC 42 Will save or be stunned for 1 round and staggered for 1d4 rounds thereafter.
Infernal Contagion (Su) On a critical hit with his hooves, Buer may afflict the target with a cursed disease (DC 38 Fortitude) that reduces Wisdom or Charisma by 1d6 each day until greater restoration is cast.
Summon Devil (Sp) 3/day, Buer may summon 1d4 pit fiends or 1d6+2 erinyes with a 100% chance of success.
