Tlazoltéotl — Eater of Filth, Confession and Purification

Tlazoltéotl is a goddess of impurity and cleansing whose power works in both directions: she can bring disorder and disease, yet she can also remove them. Her name is tied to tlazolli, “filth” or refuse, and Nahua sources connect her with confession of sexual transgressions, fertility, physicians and midwives. She is not merely a caricatured “goddess of vice.” Her divine logic is that what is hidden, embodied and dangerous must eventually be faced, named and transformed.

Traits
Divine Resistance (4/Day). If Tlazoltéotl fails a saving throw, she can choose to succeed instead.
Eater of Filth. At the start of each turn Tlazoltéotl may remove one disease, poison, curse or condition from a creature within 60 feet. If the creature is unwilling, it makes a DC 33 Charisma save.
Nothing Hidden Forever. Creatures within 120 feet have disadvantage on checks made to maintain a deliberate lie after Tlazoltéotl directly asks them about an action they personally committed.
Midwife of Return. Healing received by allies within 120 feet restores an additional 10 hit points once per turn.
Divine Spellcasting
Wisdom is her spellcasting ability (spell save DC 33, +25 to hit).
At will: detect thoughts, lesser restoration, remove curse, speak with dead.
3/day each: greater restoration, heal, modify memory, true seeing.
1/day each: mass heal, power word heal, true resurrection.
Actions
Multiattack. Tlazoltéotl makes three Black-Cotton Cord attacks.
Black-Cotton Cord. Melee or Ranged Spell Attack: +25 to hit, reach 20 ft. or range 180 ft.; Hit: 70 (6d8 psychic plus 8d8 necrotic) damage, and the target cannot benefit from advantage until the start of Tlazoltéotl’s next turn.
Confession Without Witness (Recharge 5–6). Up to three creatures within 120 feet make a DC 33 Wisdom save. On a failure each becomes Incapacitated until the end of its next turn and experiences a private vision of one harmful act it has refused to acknowledge. Tlazoltéotl learns only whether the creature accepted or rejected the vision, not the memory itself.
Consume the Corruption (1/Day). Tlazoltéotl removes all curses, diseases, poisons and magical conditions from creatures of her choice within 120 feet. For each condition removed she gains 10 temporary hit points, to a maximum of 200.
Legendary Actions
- Read the Face. Learn whether one creature within 60 feet is currently Charmed, cursed, poisoned or diseased.
- Black-Cotton Cord (Costs 2). Make one Black-Cotton Cord attack.
- Purge (Costs 2). One ally within 60 feet immediately repeats a saving throw against an ongoing effect.
- Return What Was Hidden (Costs 3). One Invisible, disguised or shapechanged creature within 120 feet makes a DC 33 Charisma save or becomes visible and assumes its true form until the end of its next turn.
Mythic Trait — Tlaelquani, Eater of Impurity
Once per encounter at 0 hit points, Tlazoltéotl consumes every harmful magical condition within 60 feet, ending them, and returns with 420 hit points. Hostile creatures whose conditions were removed take 22 (4d10) psychic damage as the burden is transformed rather than merely erased.

Worship in the Living World
Tlazoltéotl’s cult contains an apparent contradiction that is actually its centre. She is associated with the creation or embodiment of impurity and with its removal. The National Museum of Anthropology describes her as a deity of Huastec or Tének origin adopted into Nahua religion, invoked in confession of sexual transgressions, and understood as both causing and curing illnesses.
She is also a patron of physicians and midwives. The campaign therefore treats purification as practical as well as ritual: confession without treatment is incomplete, and treatment that ignores social harm is incomplete too.
Mythic Identity
Tlazoltéotl does not require people to be spotless. She requires dangerous things to stop hiding. Disease, desire, guilt and bodily processes all become more destructive when denied or exploited. Her priests are at their best when they can hear what others cannot safely say, then separate confession from spectacle.
Divine Realm — House of the Four Confessions
This campaign estate has four courts: earth, water, steam and moonlight. Visitors may leave a burden in one court only by taking responsibility for its consequence in another. Nothing spoken there can be repeated outside without the speaker’s permission unless silence would directly endanger another person.
What Is Tlazoltéotl Doing Now?
Famine has made private wrongdoing easier to excuse and harder to expose. Food is traded for favours, healers are overwhelmed, pregnancies become more dangerous under malnutrition, and powerful households can hide misconduct behind the claim that “everyone is desperate.” Tlazoltéotl rejects that excuse.
She is strengthening midwives, physicians and trusted confessors. Her shrines are becoming rare places where people can report exploitation without immediately becoming public spectacle. At the same time, she is exposing people who weaponise confession—priests who sell secrets, nobles who force admissions, and healers who create dependence by withholding cures.

Divine Relic — Black and White Spindle
This original game relic spins black fibre when a curse, disease or concealed magical compulsion is present within 30 feet. Once per day its bearer may cut the thread to grant one creature a new saving throw against that effect with advantage or a +4 sacred bonus, depending on edition.
Regional Effects
- Herbs Grow in Waste. Useful medicinal plants appear in neglected ground near favoured shrines.
- Confessions Stay Quiet. Deliberate magical eavesdropping on a sincere private confession produces only rushing-water sounds.
- Hidden Sickness Marks Cloth. Clean white cloth laid over a diseased creature develops a temporary dark pattern indicating whether the sickness is mundane, poisonous, cursed or supernatural.
Relationships
Xochiquetzal: both intersect with fertility, maternity and feminine ritual, but Xochiquetzal emphasises flowering life, beauty and craft while Tlazoltéotl governs impurity, confession and purification.
Itzpapálotl: some traditions and museum interpretation preserve overlapping mother-goddess names and identities. The campaign keeps them distinct while acknowledging those historical overlaps.
