The Plague Warped Flesh Golem is a massive, hulking construct of stitched-together cadaver parts swollen with festering sores and oozing pustules. Its mottled skin pulses with sickly green and black veins, and its eyes glow with a malevolent necrotic light. Bits of corroded metal and bone pierce through its flesh, reinforcing its grotesque frame. A putrid miasma of decay and rot clings to its form, wilting nearby plants and sickening those who approach.

Behavior:
Driven by a single-minded rage and a malevolent semblance of intelligence, the Plague Warped Flesh Golem relentlessly seeks out living beings to infect and destroy. It moves with surprising speed for its size, slamming its enormous fists to pulverize foes and spreading disease through each crushing blow. It rarely stops to rest, fueled by the dark magic animating its twisted form and a deep-seated hunger to propagate its plague.

Habitat:
These abominations are typically found in blighted wastelands, cursed crypts, or the infernal laboratories of demon lords and necromancers. They thrive in environments saturated with death and decay, where their putrid aura seeps into the land, corrupting soil and water alike.

Modus Operandi:
The Plague Warped Flesh Golem approaches battles with overwhelming force, using its thick hide and regeneration to shrug off attacks. It focuses on melee combat, striking with heavy slams that inflict both physical devastation and debilitating disease. Upon killing or severely wounding foes, it can unleash a noxious burst of virulent gas, sickening and blinding those caught within its range. When wounded, it may enter a berserk state, attacking indiscriminately until destroyed or incapacitated.

Motivation:
Created and bound by dark magic or demonic will, the Plague Warped Flesh Golem exists to serve as a living weapon of pestilence and destruction. Its primary purpose is to spread plague and chaos, undermining civilizations and weakening armies before they can mount resistance. Though it lacks true sentience, its creation imbues it with an insatiable drive to propagate death and suffering wherever it treads.

Traits

Berserk Plague Core. When the Plague Warped Flesh Golem drops below 50 hit points, it must succeed on a DC 15 Constitution saving throw or go berserk. While berserk, it makes melee attacks at random within reach (including allies) and gains resistance to all damage.

Plague Aura (Aura 10 ft.). At the start of each of the golem’s turns, each creature within 10 feet must succeed on a DC 18 Constitution saving throw or contract a magical disease. The disease reduces the target’s hit point maximum by 5 (1d10) every 24 hours until cured.

Constructed Resilience. The golem does not need to eat, drink, sleep, or breathe.

Lightning Overload. When subjected to lightning damage, the golem becomes supercharged until the end of its next turn. While supercharged, its slam attacks deal an extra 1d10 necrotic damage and it emits a sickly green light in a 10-foot radius.


Actions

Multiattack. The golem makes two slam attacks. If equipped, it may replace one slam attack with a Whirling Bone Saw attack.

Slam. Melee Weapon Attack: +10 to hit, reach 5 ft., one target.
Hit: 18 (2d10 + 6) bludgeoning damage plus 9 (2d8) necrotic damage.
If the target is a creature, it must succeed on a DC 18 Constitution saving throw or become diseased for 1 minute. While diseased, the target cannot regain hit points. It may repeat the saving throw at the end of each of its turns.

Putrid Burst (Recharge 5–6). The golem releases a blast of diseased gas in a 20-foot radius. Each creature in the area must make a DC 18 Constitution saving throw or take 27 (6d8) poison damage and be blinded until the end of its next turn. On a successful save, the creature takes half damage and is not blinded.

Whirling Bone Saw (Optional). Melee Weapon Attack: +10 to hit, reach 10 ft., one target.
Hit: 21 (3d8 + 6) slashing damage plus 13 (3d8) necrotic damage.
The target must succeed on a DC 18 Constitution saving throw or suffer deep festering wounds: healing effects are halved, and it has disadvantage on Constitution saving throws until the end of its next turn.
While supercharged, the bone saw deals an additional 1d10 lightning damage.


Tactics

The golem charges into the frontline to disrupt enemy healing and spread disease. It synergizes well with Marbas’s surgical madness, creating zones of attrition. Easily mass-produced by Marbas’s Flesh Architect reaction.
